One On One Diabetes Education
Patients can benefit greatly from the 1:1 sessions because the plan of care is individualized. The educator uses a comprehensive diabetes book to lead the patients through the education, discussing topics such as diabetes process, blood sugar monitoring, preventing complications, medication management, exercise and nutrition. A flexible meal plan is designed specifically for each patient. Patients can also ask questions or express feelings they may not feel comfortable talking about in a group setting.
Education of the following topics is provided to patients during individual sessions:
- Initial diabetes education
- Diabetes refresher
- Meter instruction
- Insulin instruction or adjustment
- Carbohydrate counting
- Pre-diabetes (patients with higher-than-normal blood sugars, but not high enough to be considered diabetes at this time)

NCQA Goals & Attainment
Overall Results:
To qualify as a Diabetes Physician Recognition Program (DPRP), Clay Platte Family Medicine Clinic was required to conduct a readiness evaluation prior to applying for their DPRP status. Clay Platte Family Medicine Clinic randomly selected 25 diabetic patients per physician between the ages of 18 and 75. The overall score for this readiness survey sample was a perfect 100 out of a possible 100 points. Since this score does exceed the 75 point threshold, this sample could be submitted to obtain recognition for NCQA’s DPRP (see Table 2).

Conclusions
The results of this analysis demonstrate that Clay-Platte Family Medicine Clinic has met all criteria needed for NCQA’s DPRP recognition and was subsequently granted the recognition in April 2011.
Group Class Education
Group class education is geared towards patients who seek information on diabetes, weight management, heart health and overall healthy lifestyle management. The 1 hour online class is offered once a month by our diabetes educator. These classes are all held online, so you can participate in the comfort of your own home. Activities, visuals and real-life situations help the group learn from one another and think critically to solve problems related to their diabetes. Patients are also encouraged to set up a 1:1 appointment with the diabetes educator to design a specific meal plan.
Patients are given numerous handouts, booklets and instructions to manage their diabetes, as well as contact information for the diabetes educator when they receive education.

Continuous Glucose Monitoring Systems
Patients wear the Freestyle Libre Pro for 14 days, after which providers can review the data and use the results to uncover glucose patterns and optimize patient therapy. Based on the detailed glycemic profiles collected from the Freestyle Libre Pro, providers can better tailor diabetes treatment programs for each patient. This may be particularly helpful for patients who experience inconsistent high and low glucose levels, who experience hypoglycemia unawareness and who generally desire better control, as well as for women with gestational diabetes (pregnant women with diabetes).
The Freestyle Libre Pro is attached to a tiny glucose sensor inserted just under the skin. During the course of 14 days, the sensor automatically measures and stores glucose values during daily activities like work, sleep, eating and exercise. After the recording period is completed, the patient returns to the provider’s office where the device is removed and downloaded. The provider can then generate and interpret detailed glucose reports to determine changes to the patient’s therapy.
